All rates for calendar year 2023.
ELIGIBILITY: Health and welfare plans are available to all employees who work a minimum of a 40-hour work week in a budgeted position on a regular basis as defined in the City’s Personnel Policies and Procedures.
NEW HIRE ELIGIBILITY: Benefit Eligibility starts on the first of the month following 60 calendar days of service.

OPEN ACCESS PLUS (IN-NETWORK) (OAPIN) This program allows for coverage for care In-Network only, employees and their Dependents to pay a portion of the Covered Expenses for services and supplies. That portion is the Copayment, Deductible or Coinsurance.
HIGH DEDUCTIBLE HEALTH PLAN/HEALTH SAVINGS ACCOUNT (OAP [HDHP/HSA]) This program allows for coverage for care In-Network and Out-of-Network. Employees and their Dependents may be required to pay a portion of the Covered Expenses for services and supplies. That portion is the Deductible or Coinsurance. Employees who have the High Deductible Health plan are eligible to designate a dollar amount of their paycheck to a with a tax-free Health Savings Account (HSA) that employees can use to help pay for some of their covered health care costs now and into the future.
HEALTH INSURANCE OPT-OUT The City offers an alternative option for active employees who have proof of other qualifying health coverage (individual, group or Medicare Parts A & B.) Employees may opt out of the City's health plan and receive $195 a month, paid on a bi-weekly basis.
EMPLOYEE ASSISTANCE PROGRAM (EAP) through Evernorth A service of personal advocates that work with employees and their household family members to help them resolve issues they may be facing, connect them with the right mental health professionals, direct them to a variety of helpful resources in your community and more. Employees and their family members do not pay for EAP counseling services. Should additional counseling be necessary, EAP will work with the employee's current health provider for coordination of service and can assist in a referral to the best local resource at the lowest cost to the employee. (Eligibility on first day of employment)
TERM LIFE INSURANCE/AD&D through New York Life Coverage based upon annual salary (in increments of $10,000). Maximum coverage of $50,000. Supplemental insurance available at employee cost. Optional term life only is available for spouse and dependent children at employee's cost.

RETIREMENT PLAN FOR GENERAL EMPLOYEES The City of Maitland General Employee's Money Purchase Plan 401(a) is a defined contribution plan.

EXEMPT EMPLOYEE DEFERRED COMPENSATION PLAN CONTRIBUTION The City contributes an amount equal to 3% of base pay for exempt employees to the 457 Plan. Starting with their first payroll.
DEFERRED COMPENSATION PLAN 457 and ROTH Employees are eligible to participate and contribute their pretax or post tax dollars into the 457 (b) plan. (Eligibility starts on the first of the month following 60 calendar days of service.)
COMPRESSSED SCHEDULE Established a schedule of four (4), ten (10) hour days, on a workweek of Monday through Thursday and closed on Fridays. Public Works has a (30) thirty-minute lunch, other General employees have a (1) one-hour lunch.
PERSONAL LEAVE Employees accrue 130 hours annually with an increase of 40 hours at 5 and 10 years of service. (Eligible at 6 months)
HOLIDAYS Employees observe 8 paid holidays annually.
SICK LEAVE Accrue 100 hours annually. (Eligible at 6 months)
MILITARY LEAVE Upon presentation, and confirmation, of official orders, the City will grant an extended leave of absence to any employee called to "Active Service" to perform duties in the uniformed services during a National, State or other officially declared emergency or disaster. All regular full-time employees are eligible for full pay and benefits within the first thirty-day period from the date of activation with the confirmation of official orders. Thereafter, for a period of up to eleven months, the city will supplement the difference between an employee's military base pay compensation and the employee's regular rate of City pay (including any incentive payments).
COMPASSIONATE LEAVE The City's Compassionate Leave Donation Program is a voluntary program that provides employees with potential leave hours should a catastrophic illness, accident, or other extenuating circumstances occur. The program allows full-time employees the opportunity to donate accumulated personal leave hours to full-time employees who have exhausted all paid time off.
UNIFORMS City provides uniforms (if applicable).
TUITION REIMBURSEMENT PROGRAM Maximum of $1,600 per fiscal year may be reimbursed for qualifying formal academic education.
DIRECT DEPOSIT PROGRAM A direct deposit program is available to all City employees, which provides for deposits directly to the financial institution and account of the employee's choice.
SECTION 125 PLAN The City offers a Flexible Benefits Program, which allows employees to pay qualified benefit premiums before any taxes are deducted from their paycheck.
FLEXIBLE SPENDING ACCOUNTS (FSA) Allows employees to set aside money from their paycheck before income taxes (Federal, Social Security, Medicare, state and local taxes, if applicable) are withheld. This money is available to pay for eligible expenses, such as your medical deductibles and copayments, prescriptions, dental expenses, eyeglasses, contact lenses and other health-related expenses that are not reimbursed by your health plan. Health FSAs are subject to a “use it or lose it” rule. Any unused funds at the end of the year will automatically be forfeited.
LEGAL SHIELD This is a Prepaid Legal service available to employees, at their expense, which provides for legal needs to include IRS audits, consumer problems, trial defense, etc.
SUPPLEMENTAL INSURANCE A variety of supplemental health coverages including Cancer, Disability, Accident, etc., are available from AFLAC. These coverages would be at the employee's cost.
OTHER BENEFITS Additional benefits, which are not listed in Section VII of the City's Personnel Policies and Procedures Manual, include Wellness Programs, Safety Incentive Program, and other employee-enhanced programs.
