Comprehensive Development Plan Amendment Submittal Checklist

Applicant Checklist Required for Submittal Sufficient Insufficient
  Complete Application: On-line application has been thoroughly completed.    
  Fees:  Application and Review Deposit fees have been paid.    
  Property Owners: List of the names and mailing addresses of all property owners within 500' of the property proposed for land use amendment taken from Orange or Seminole Counties.    
  Authorizations: Name, address and phone number of any agent authorized to represent the property owner(s) of the property proposed for land use map amendment.  For a text amendment, the applicant may provide a notarized letter of authorization for an agent to represent the property owner whos signature appears on the application form.    
  Vicinity map depicting surrounding streets and existing zoning and land use on the site and surrounding area (within 500 feet).    
  Boundary survey and legal description for each parcel of property subject to the application but owned by separate and distinct persons, corporations, partnerships, or other legal entities; and prepared by a surveyor registered by the State under a surveyor's seal and certified to the City. If applicable, provide the gross and net land area which may be provided on a separate sheet.     
  Analysis: Complete analysis of the proposed amendment (sections a through f of the procedures manual).    
  Statement of Appropriateness: A general description of the proposed amendment to the comprehensive development plan, explaining why the amendment is appropriate or necessary.    
  Phasing plan: Estimated timetable and method of financing for each public facility improvement necessitated by the proposed comprehensive development plan amendment.
